We will send you a portfolio packet of-up-to date information on Cord blood banking, the packet will include information about our services and the necessary enrollment materials.

How to enroll
To enroll in LifeLine Cryogenics Cord Blood Banking Service, you must complete and sign all the enrolment forms listed below. If you do not have all the necessary forms, you can download them on our website.
  • Lifeline Cryogenics Enrollment Form
  • Lifeline Cryogenics Enrollment Agreement
  • Material History Form
  • Informed consent for infectious disease testing

Mail or fax the complete list of enrolment forms to Lifeline Cryogenics, along with a $300.00 deposit.

Upon receipt, the complete list of enrollment forms and $300.00 deposit, a cord blood collection kit will be sent federal express 3 day service.

Note: For parents enrolling within 3 weeks of due date, expedited delivery services is offered at additional charges.

Once enrolled with LIFELINE CRYOGENICS
Bring the Lifeline Cryogenics Cord Blood Collection Kit with you to the hospital. (Upon admission to the hospital, let your doctor and nurse know about the collection kit.)
Mother's Blood
Upon admission to the delivery area, have the mother's blood drawn in the three tubes provided in the kit. The tubes must be dated and labelled correctly by the individual drawing the blood.
Collection of the Cord Blood
After the baby is born and the cord is clamped and cut, the cord blood will be collected by the gravity using our especially designed cord blood collection bag. The collection of cord is a quick and painless procedure to both mother and child.
After the collection of cord blood
Call Lifeline Cryogenics toll free at (866) 967-2796. We are available 24 hours a day. We will then arrange for our dedicated medical staff to transport your cord blood specimen directly to Lifeline Cryogenics facilities. The cord blood specimen must be picked up and delivered to our facilities within a 24-36 hour time window. The cord blood specimen must be kept at room temperature. Do not refrigerate.
Arrival at Lifeline Cryogenics
Upon receipt of the cord blood at our facilities, our technologist will test and process the cord blood. During the processing, the cord blood will be tested for cell viability, total nucleated cell, blood type and blood culture. Once the processing is completed, the cord blood specimen will be frozen in our state-of-the-art control-rate freezing machine.

Once the cord blood specimen is frozen, we will contact you and let you know.

  • Store the kit at room temperature to avoid temperature extremes.
  • Do not leave Cord Blood Collection Kit in your car
  • To maintain sterility of the Cord Blood Collection Kit, the seal must not be broken until time of delivery.
